                                I am now in my 50s but am still yet to get rid of my fear of the dentist. I am able to go to appointments for the small things but am not keen at all on anything too painful. One of my front teeth is now in a bad state due to my fear, and the dentist has said that I will need to get a denture. I have said I don&#8217;t want this as I fear it falling out, and am wondering if an implant could be a good choice. Is this procedure able to be carried out with the patient fully asleep, or does this carry too many complications?

                                Hi, many thanks for your email. The main options for a gap are 1: denture 2: bridge 3:implant or 4:nothing at all. if the denture is a worry, the two fixed options would be a bridge or an implant. If feesible my advice would always be to go for an implant as its the longest lasting of the two options and most conservative.</p>
<p>The procedure of an implant could be performed under IV <a href="http://www.cosmeticdentistryguide.co.uk/articles/sedation-dentistry.html">sedation</a>, which would mean you&#8217;re unaware of the treatment while it is taking place. </p>

My dentist has now informed me that I will need to get dentures, but I am not particularly keen on this. Are there any other treatment options, or will my teeth just continue to get worse if I don&#8217;t get dentures?

I am quite a keen coffee and tea drinker, and I also smoke, all of which has worked to stain my teeth a tarnish their natural colour.                                 Hi, many thanks for your email. The ideal time to whiten your teeth is once the implant itself has been placed. There is usually a healing time of around 2-3 months before the final crown can go on the implant. During this healing time would be a perfect time to then have your teeth whitened.                                 Dear Doctors,<br />
I live in South Africa and in a few days time I am due for a scary extraction of my left over teeth (mainly the front ones) prior to puttin g implants in my mouth.<br />
I am so scared that half way through the extraction I will tire out and panic&#8230;<br />
Originally the Government Hospital offered me <a href="http://www.cosmeticdentistryguide.co.uk/articles/sedation-dentistry.html">sedation</a>, but this being an overbooked service thay have now suggested we swap it for a a heavy sleeping pill which will keep me half awake but not fully sedated. If I waitr another two months I could use the seadtion option instead but I amn not sure if I should do that, also I am scared that getting used to temporary dentures will depress even further!! I am in need of compassionate counselling and advise. Please reply soon!

                                Sorry to hear of your unfortunate situation.  I think you will do well with the heavy sleeping pill.  You will be very relaxed and the procedure will feel as if it went quickly.  When you are ready for the implnats wait until they can schedule for <a href="http://www.cosmeticdentistryguide.co.uk/articles/sedation-dentistry.html">sedation</a>.  <a href="http://www.cosmeticdentistryguide.co.uk/articles/sedation-dentistry.html">Sedation</a> is a better option for the implant appointment.  Don&#8217;t let the tempoaray dentures depress you.  Instaed look forward to the day you will no longer need the denture and how good that will make you feel.  Dr. Lemongello

                                hi many thanks for your email, the grafting material is dependant on the site to be grafted and the size of the graft needed. if the size of the graft is large then my advice would be to use your own bone as this will integrate far better than artificial graft material. if the size of the graft needed is small you could use artificial bone mixed with your own bone to graft the site which research has shown is just as good. it is all dependant on the site and the comfort zone of the surgeon. a common artificial graft material is bio-oss it is derived from bovine bone. </p>

Depending on the implant type and the additional parts required to create the crown for the implant you can expect to pay at least the same amount for the crown, and likely more. Consult a Periodontist on the implant placement and let them recommend a general dentist to restore the tooth. You do not want to shop implants by price.                                 The advantages of a bridge are ease of placement and it may be a very good option especially if the teeth needed to hold the bridge also need to be restored. Life expectancy varies but should last a long time. The disadvantage is if the teeth needed to hold the bridge are healthy and do not need to be restore. In this case you are crowning them only to hold the bridge. You can always have the bridge changed in the future and add implants but there may not be a need once the bridge is placed and as long as it remains healthy. <br />Dr. Lemongello
