Hello Doctor
one of my front teeth has been moving down with time such that my two front teeth are now not on the same level. yet they were on the same level before. i noticed this 6 months ago. the tooth is not hurting yet i realise it is moving longer by more inches. at first it wasnt significant but now it is very visible. it is growing longer and longer. what do i do?
please advise me before it moves further. thank you for your time

On Monday 5/23 my 10 year old son got his front tooth knocked out of the socket at baseball. It was completely out but hanging by a small string of soft tissue. Within 45 min it was back in place and bonded. On Tuesday 5/24 we went to the pediatric dentst and he looked at it and said it looked good but he would need a ododontist. We have an appt. on Tuesday 6/1. By the the ER put the tooth back and my orothdontist bonded it. My pediatric dentist tells me he will definately need a
root canal. People who have been through the same thing said if the tooth takes then he shoudn’t need a
root canal. Others said he should have had one right away. Can you please give me your opinion? Thanks

Most often, it will have to have a

I would if you could tell me is their a limit to the amount of times you can either use home whitening kits or have in practice sessions, I had a teeth whitening session at a dental practice last year and loved the results and even though they are still white I would like them to be brighter. I’ve been hearing a term lately called ‘bleachorexia’ which is basically the over use of teeth whitening products, what would you deem to be over using. I had my last teeth whitening session around this time last year, am i ok to receive another session of teeth whitening?

There’s no mild way of putting this, but I have awful, awful breath. It has been a hindrance all my life and I have tried a number of different treatments and life changes. I am very conscientious when it comes to oral hygiene brushing my teeth twice a day, using mouthwash and flossing but this has done little to help with the smell. I then changed my diet at the recommendation of one dentist but this did little also. What are your recommendations doctor, I am willing to try anything as it truly is ruining my life.

There is usually a cause to breath malodor. Systemic disease can be a cause, such as diabetes. Also conditions like gastric refux can play a part. Additional causes may be related to old fillings, or underlying gum disease. Even though your hygiene is good this could be a factor. Some mouthwashes dry out your tissue if they contain high concentration of alcohol. If none of these apply you may want to try products available designed specifically to treat breath malodor. Products like Oxyfresh or Breath Rx are good products that may help. Also, don’t overlook tongue hygiene. Use of a tongue scaper is invaluable. Dr. Lemongello

I have always disliked my gums, which are very noticeable and make my teeth look tiny. Not only that my lips are prone to being very high above my teeth, which makes me look even more gummy. I have heard of gum contouring but could you tell me what this actually entails. Would i be awake for the procedure and how long does it usually take for any swelling to subside. On top of that is their anything you can recommend for my lip problem as well?

Reshaping of the gum tissue with laser is a painless procedure and the healing time is very quick. It is something that is done with only local anesthetic. It would be your best option. hope this helps

i recently got into an accident and due to this incident my two front teeth are missing and I am desperate for a replacement. My dentist says i will have to wait for the swelling etc to go down and then he can explore my options. What would you recommend as being the best treatment for me? I’ve always in the best dental health and before this have never even had a filling, so my teeth and gums are strong. Do you think i would be a successful candidate for implants even with the trauma?
also i have heard of snap-it or snap on smiles would they be ok for me to wear in the meantime? i know they wouldn’t be a permament option but im just so embarrassed at my appearance right now. How much are these usually?

Sorry to hear of your misfortune. You do have to wait for some healing prior to starting treatment. Implants should be a good option. Discuss with your dentist temporary options during the healing phase. Snap on smile might be an option but others are available as well. Dr. Lemongello

I am a 47 year old women with periodontal damage and I am soon to be loosing a tooth as my NHS dentist has recommended that it be extracted before the decay spreads. My question concerns the after treatment and how best to replace the missing tooth, which is in a really obvious place at the front on my mouth. I have been looking into partial dentures, bridges etc but what would you recommend.
I like the idea of implants just because they seem more permanent, but I suppose with my gum disease I would have to wait for it to settle before I got implants.
Would this be the case doctors, and have you used implants on other patients who have suffered from periodontist?

Implants are the best alternative in most cases. Bridges and partials work fine too but have their advantages and disadvanatges based on the teeth that hold them. Implants like teeth can be affected by periodontal disease but that doesn’t mean they are not the best choice. Consult with your dentist on what is best for you in your situation. Dr. Lemongello

i have very crowded teeth on my top jaw and my dentist tells me a very narrow arch. I feel that orthodontics are my best option but was wondering which would be the best system for my current condition. I have looked at invisalign, damon braces and lingual and am a little stuck for which to choose. Which would you think is the most effective in concerns to my case and how would it work? Also, i know generally you would have to see me to know this, but what is a general time span for the most effective brace?

Certainly orthodontics are an option and any of these treatment modalities will work. If you are looking to change the shape of your teeth or the color, orthodontics will not address these issues, it will only give you straighter teeth. Cosmetic dentistry is another option which would allow you to do all the above in as little as 3 weeks!

I have recently chipped my front tooth, after a cycling accident and have had it checked out by my NHS dentist who is more than happy with the stability of the tooth, and doesn’t fear any infections etc
What would be the best way to rectify the appearance of this tooth? The tooth is chipped in the bottom left hand corner and has a slightly sharp edge. I have looked at veneers and cosmetic bonding which would you think is the best treatment for me and which is the most long lasting in your opinion?

Porcelain veneers and cosmetic bonding both have advantages and disadvantages. Cosmetic bonding is a good choice if a small amount of tooth needs to be replaced. The disadvantage is that it doesn’t last very long and may need to be replaced every 3-5 years. Porcelain veneers are stronger and will last longer but are more invasive and more expensive.
